New Patient Information

Thank you for your interest in our practice. Patients new to our practice requesting an appointment for a consultation can call our office at 713-794-0700. Alternatively, you can e-mail us supplying the required information and we will call you to arrange an appointment.

Kindly remember that in certain circumstances, you will need to get prior approval from your insurance company before an appointment can be made. If not, there is the possibility that the cost of the consultation would be based on “out of network” benefits. Many patients are aware of this and are willing to pay the increased charges.

New patients that have been scheduled an appointment can download both the Medical History Form and Insurance Form and bring them with you the day of the appointment. This may reduce the amount of time filling out forms in the office.

Additional office forms available for review are listed below. Reviewing the forms may answer questions you have and avoid the need to call the office.

For patients seeking a dedicated “Second Opinion”, kindly download and fill out the Second Opinion Form and fax it back to our office. For your convenience, a fax cover sheet is also attached.

    Hello, My aunt is very sick she had hepatiis c and did not know it so her liver got damaged. She was diagnosed about 2 yrs ago. She is in very bad shape now and all the doctors from San Marcos Tx have told her that theres is nothing that they can do for her anymore. She is a young woman with a 3yr old son. She has medicaid. All they also told her that she didnt qualify for a liver transplant. Please contact me this is our only hope. She is very sick..

    I have a dear friend who was Dx'd w/liver disease a few yrs ago. She has had several incidents of severe itching which has turned into hives. Her drs are leaning toward allergic reaction to meds; but we are concerned it is related to the Hep C/liver disease. We know itching is common in liver disease, but does liver disease cause hives as well? We certainly appreciate your input as we need to resolve/get to the bottom of this situation. Many thanks!!!

    I AM 44 YEARS OLD AND HAVE BEEN HAVING PROMBLEM WITH MY AMMONIA LEVEL GOING UP IT HAPPEN TO ME TWO YEARS AGO BUT MY ENZEM SHOWED OKAY FROM TIME TO TIME I KNEW WHEN IT WAS UP BECAUSE I CAN SMELL IT IN MY NOISE SO I TOLD MY DR. AND HE REPEAT MY LAB AND IT WAS UP AGAIN CAN SOMEONE PLEASE HELP ME IF MY ENZEM SHOW OKAY THE LIVER OKAY THEN WHAT IS CAN BE CAUSEING MY AMMONIA LEVEL TO GO UP SO HIGH

    I had a gallbladder surgery(lap.) in 1993,I have had to have 6 or 7 ERCP.'s since then,I have 13-24 surgical clips in my liver area.I have been in ICU several times due to infection,they never know where or what it is from.My ALT & AST have been up to 1500,&1800sed rate 550 .Please write me back!!!Thanks;Diane Fry

    hi my name is maria calderon and my husband was told he had colon cancer he is now in remission thank god doc. davila @ m.d. anderson now told us he has cirrosis i work for a*med hospice and one of my nurses told me that u were a good doc to speak with he is 52 yrs and has only medicare for insurance he is getting help by m.d. anderson with other things please write back i would like to c if we could come and speak with u please respond /maria calderon
